Driving Logs and Affidavits

Ohio 50-Hour Affidavit   (Supervised Driving Log)

Required for teen students to document the 50 hours of supervised driving completed with a parent, guardian, or qualified licensed driver before taking the Ohio BMV road test.

24-Hour Affidavit

Required for adult students completing the Abbreviated Adult Program to document the 24 hours of supervised driving completed with a qualified licensed driver before retaking the Ohio BMV road test.

Training Agreements

Behind-the-Wheel Training Agreement

Required agreement for students enrolled in the Teen Driver Education Program for Behind-the-Wheel only training.

Abbreviated Adult Training Agreement

Required agreement for students enrolled in the Abbreviated Adult Program.

Certificate Requests

Request for Duplicate Certificate

Use this form to request a replacement copy of a previously issued certificate of completion.
